Transitioning To Memory Care

Difficult Decisions, Positive Outcomes…

The decision to change residences is never an easy one, especially when entering a new phase of life. At Green Meadows Care Home we strive to make that decision for your family a positive one.

Often families just like yours approach us after an unexpected stressful event. Considering all the decisions that need to be made when these life altering events happen, it can quickly become overwhelming.

If you find yourself in this very situation, you’re not alone, we’re here to help. However, if you’re looking ahead in case an assisted living home becomes a necessity, we’re here to help you plan a future move as well.

Preplanning allows you to find the right location and the dedicated memory care team your family deserves free of stress and urgency.

Understanding that most families don’t know where to start, we’ve created the 4-M’s Checklist. What are the 4 M’s you ask? Mobility, Memory, Meals and Medication.

In addition to the 4-M’s Checklist, we offer a complimentary consultation with our owner, Diana Nicorici, RN, BSN. Diana will listen carefully to your questions and concerns, then work with you to create an individualized plan to meet the needs of your family’s valuable member.

Should you discover that Green Meadows Care Home is not the best fit for your family member’s needs, we are happy to assist you in finding the appropriate care home in your preferred geographic location.
